Fee Schedule
CME includes review of medical records/films, the exam and the written report.
A prepaid fee of $4,600 is required.
Records & prepayment must be received no later than 4 weeks prior.
No additional charge for videographer or court reporter. Plaintiff attorneys are welcome.
If the exam is canceled with less than 2 weeks notice, & WILL NOT BE RESCHEDULED, there will be a $2,500 disruption fee. If the exam is going to be rescheduled, there would be no refund and we would then invoice an additional $2,000 for the new CME date. CME’s canceled with less than 72 business hours notice or if the plaintiff is a no show, there will be no refund.
There is a minimum prepaid fee of $3,000 required and will need to be here in the office within 72 hours of the deposition. Any additional time will be invoiced at $500 per 15 minutes. There is no additional charge for video deposition.
There will be a $1,000 disruption fee if canceled within less than 72 business hours notice.
$2,000 per hour, portal to portal. A prepaid deposit of $8,000 is required and will need to be here in the office prior to his appearance. This includes review of records, and any additional time will be invoiced at $500 per 15 minutes.

Compulsory Medical Examination

Prepaid fee is required. Records review with 15-minute phone conference $2,500. Records review with written report $3,500. Records and prepayment must be received prior to scheduling. If canceled with less than 2 weeks’ notice, there will be a $500 fee for preparation of records.
Conference
Prepaid fee of $500 is required for the first 15 minutes. Any additional time will be invoiced at $500 per 15 minutes.
Excess Records Fee
One banker’s box of records included in the $4,600 CME fee. Any records sent more than this will be billed at $250 per banker’s box.
